CHA Eligiblity

For a business and sole proprietors to be eligible for the Community Health Alliance plan, they must meet the following criteria:

The Business must:

  • Operate on a full-time basis and cannot be a hobby or seasonal business.
  • Have at least one employee and no more than 50 employees.
  • Meet income guidelines which are based on the median wage of employees wanting to participate.
  • Be located and operate in Baker, Union or Wallowa County.
  • Provide the Oregon Quarterly 132 .
  • Have not offered employer based commercial health insurance to some or all of its employees in which the employer contributed at least 50% of the cost of premiums in the past 12 months.
  • Offer the plan to all uninsured eligible permanent employees. (employer must offer the plan but not all employees have to participate)
  • Dependant coverage must be offered to uninsured dependants that reside in the coverage area. (The employer is not required to pay either portion of dependant’s share of coverage.)
  • Employer must agree to participate in the plan for at least 12 months.

 

For an individual employee to be eligible to participate in the Community Health Alliance plan, he or she must meet all of the following criteria .

The Employee must:

  • Be employed by a Qualified Employer
  • Live or work in Baker, Union or Wallowa County.
  • Be an active, paid permanent employee.
  • Have been employed as a permanent employee for 13 weeks with the qualified employer.
  • Does not qualify for publicly funded health care such as OHP.
