Our community health workers can help with family health care coordination, connection to and enrollment into a variety of health care options.
The Purpose of this program is to:
Healthy Families, Healthy Fun!
- Enroll businesses and individuals into Community Health Alliance (CHA)
- Help families and individuals navigate health and social services, connecting them to available health care
- Assist clients with enrollment and reenrollment processes
- Provide health education training and materials to the public and local organizations
- Inform our communities of changes in health policy that afffect our community
- Coordinate care in existing coverage programs
