OHP Plus & Standard
OHP Plus & OHP Standard
The OHP Plus Program is for people who are aged, blind, disabled, under age 19, pregnant or receiving Temporary Assistance for Needy Families benefits.
The OHP Standard program provides free or low-cost health care coverage to Oregon residents who have limited income, are ages 19 and older, and do not qualify for traditional Medicaid. To become a potential applicant, contact a CKFNEO staff to get on the OHP Standard Reservation list. If you receive notification of being drawn from the list, then that is when you need to send in an application.
Documents you may need to bring with you in order to complete the application process:
-
Proof of income for the current month. Copy of your most recent pay stub or a letter from your employer or the person who paid you (which must include employer’s contact name and phone number). You’ll need one recent pay stub for each job listed on the application.
